A Comparison of Testosterone Delivery Methods
As awareness of low testosterone continues to grow, more men are seeking evaluation and treatment for symptoms related to testosterone deficiency. Hormone optimization clinics have become increasingly common, and patients who are appropriate candidates for testosterone replacement therapy (TRT) have several options for treatment. Each delivery method offers distinct advantages and disadvantages, making it important to choose the approach that best aligns with an individual’s lifestyle, preferences, and medical needs.
Injectable Testosterone
Testosterone injections remain the most commonly prescribed form of TRT. Several injectable formulations are available and may be administered either intramuscularly or subcutaneously, depending on the specific medication and injection volume. Most regimens require administration once or twice weekly.
Because injections typically involve larger doses given less frequently, testosterone levels may fluctuate between doses. These fluctuations can contribute to a higher incidence of side effects in some patients, including elevated red blood cell counts, acne, hair loss in genetically predisposed individuals, and fluid retention. Despite these considerations, injections are often cost-effective, reliable, and highly effective when appropriately monitored.
Testosterone Pellets
Testosterone pellets provide a long-acting alternative for patients seeking greater convenience. During a brief in-office procedure, small pellets are implanted into the deep subcutaneous tissue, where they gradually dissolve and release testosterone over approximately three to six months.
The primary advantage of pellet therapy is the infrequent need for treatment, eliminating weekly or daily medication administration. However, achieving the ideal dose can sometimes be challenging, potentially resulting in over- or under-treatment. Additionally, once implanted, the pellets cannot be removed and must naturally dissolve over time if dosage adjustments become necessary.
Oral Testosterone
Oral testosterone has evolved significantly in recent years. Earlier oral formulations underwent extensive first-pass metabolism through the liver, raising concerns about liver toxicity and elevated liver enzymes. Newer oral testosterone preparations avoid this metabolic pathway, making them considerably safer for long-term use.
These newer medications may also be associated with less suppression of the body’s natural hormone production, a lower likelihood of increasing red blood cell counts, and reductions in sex hormone-binding globulin (SHBG), which may improve free testosterone levels. Their primary disadvantages are the need for twice-daily dosing due to their shorter duration of action and a generally higher cost compared with some other treatment options.
Transdermal Testosterone
Transdermal testosterone, available as gels and creams, is applied to the skin once daily. This method offers simple, noninvasive administration and can effectively restore testosterone levels for many patients.
The main drawbacks are the potential for the medication to feel messy or inconvenient and the risk of transferring testosterone to another person through direct skin contact before the product has fully dried or been absorbed. Patients using transdermal therapy must follow application instructions carefully to minimize this risk.

There is no single “best” testosterone delivery method for every patient. The ideal choice depends on factors such as lifestyle, treatment goals, convenience, cost, tolerance of potential side effects, and personal preference.
